Overview of U.S. dam safety policy and issues. Article includes national, state-by-state map of unsafe dams as a percentage of the total inspected during the USACE Phase I Inspection Program, mandated in the National Dam Safety Act of 1972. Funding for the inspection program was delayed until 1977, after the Kelly Barnes dam failure. Other failures and incidents cited: Teton, South Fork (Johnstown), Lower Van Norman, Buffalo Creek.
