Report of the Independent Civil Investigation of the March 14, 2006, Breach of Ka Loko Dam

Report prepared as part of an independent civil investigation investigation of the March 14, 2006 failure of the Ka Loko Dam. The investigation began in early August 2006, and concludes with this report. Robert Carson Godbey was appointed Special Deputy Attorney General by Attorney General Mark Bennett for purposes of the investigation. The scope of the investigation included:
1) the role and possible culpability of the State of Hawai‘i,
2) the role and possible culpability of the County of Kaua‘i,
3) the role and possible culpability of the private landowners,
4) the role and possible culpability of any other parties,
5) recommended legislation or government actions that could help prevent another tragedy, and
6) other recommendations and relevant considerations believed to be appropriate.
