A.V. Watkins Dam nearly failed in 2006 due to backward erosion piping through the foundation. This article presents the original design of the dam, conditions observed during the incident, interim measures taken to prevent the failure mode from progressing further, follow-up investigations, and factors that may have caused the incident to develop 42 years after completion of construction.
