The purpose of this report is to provide the EPA Internal Review Team's (Team) assessment of the events and potential factors contributing to the blowout from the Gold King Mine (GKM) in Colorado on August 5, 2015. This report provides the Team's observations, conclusions, and recommendations that regions may apply to ongoing and planned site assessment, investigations, and construction or removal projects at similar types of sites across the country.
