Forensic Investigation Meadow Pond Dam Alton, New Hampshire

GEI Consultants, Inc. (GEI) was retained by the State of New Hampshire Department of Environmental Services (NHDES) Water Resources Division (WRD) to conduct a forensic investigation of the failure of the Meadow Pond Dam in Alton, New Hampshire. This report provides the results of GEI's forensic investigation.

The purpose of GEIs forensic evaluation was to determine the mechanism of failure, review the design for adequacy and to determine if the dam was constructed in accordance with the design approved by the NHDES. Work conducted by GEI as part of the forensic evaluation included field investigations, geotechnical laboratory testing of soil samples obtained from the dam, and interviews with parties involved in the design, construction and maintenance of the dam.
