Abstract/Additional Information

Investigation report of breach of Silver Lake Dam, May 14, 2003. A general description of the incident and conditions as of May 23, 2003 is given in Attachment I. During the week of May 26-30 the Board was sent a package by FERC for the Board to review before visiting the site. The list of documents provided in this package is given in Attachment II. The design report for the recently constructed fuse plug was one of the most significant documents in this package. Prior to the site visit a one page listing of key elevations was made as shown in Table I.
