Incident DescriptionDam was overtopped and failed following 10-inch plus rain in less than 12 hours. Dam was an unapproved structure. It appears the dam was constructed without any engineering analysis.

Additional Remarks or UpdatesDownstream bridge was washed out. Unable to determine if it was caused by dam breach or natural flooding due to heavy rain. Dam was discovered by our staff in 2014 in aerial photos and was inspected in May 2015. The dam had evidently breached a short time prior to the inspection. We have no plans for the dam. We do not think the dam had a principal spillway.
