Incident DescriptionA sinkhole opened in the road on the dam immediately above the 120-inch diameter outlet pipe. Investigation by the Department showed that piping was occurring from the upstream side of the dam into the embankment, and water/sediment was exiting through holes, cracks and joints in the 120-inch diameter outlet pipe. Over time, 2 additional sinkholes opened in the road above the outlet pipe.
